Jason Runyan, ARNP
FOUNDER & CEO
Jason has been a Family Nurse Practitioner since 2004, with experience working in Emergency and Urgent Care prior to providing in-home, primary care. He received both his MSN and BS in Nursing from the University of Louisville in Kentucky. With over 12 years experience as a nurse practitioner, and nearly 10 in house calls, he is committed to providing excellent care and resources to patients, helping reduce or prevent hospitalization, readmission, and treating patients in the comforts of home. Jason values strong partnerships in the Healthcare community that help improve outcomes for patients in the Greater Jacksonville area. In his limited spare time, Jason is a devoted husband, proud father to his two boys, and an avid runner.
Rachel Pellum, ARNP-FN
Rachel is Board Certified with ANCC, and has been a house call primary care provider since 2013. Prior to her nursing career, she was a high school biology teacher. She became a registered nurse in 2005 and for five years specialized in med surg and oncology. Rachel was a UNF student health clinic provider from 2010-2013, and an instructor at the UNF School of Nursing. Her favorite movie is the Sound of Music, and her favorite hobby is being a wife and mother to four beautiful children.
Rick de la Cruz, MD
Dr. de la Cruz is the medical director, collaborating physician for My Choice Housecalls. His education began with a BA in biology from Johns Hopkins University, and then he earned his masters in physiology from Georgetown University, and went on to get his M.D. from Uniformed Services University of the Health Sciences (USUHS)- Military Medical School Internship in Family Medicine. He completed his residency in Emergency Medicine and is Board Certified in Emergency Medicine. His extensive experience also includes Medical mission support for SEAL Teams 2 and 4, special operations medicine, Naval Flight Surgeon, attached to East Coast Aircraft Carrier Aviation squadrons, NASA Flight Surgeon, US Navy combat veteran, Medical Director Clay County SWAT Team, and Professor of Emergency Medicine at Orange Park Medical Center. In his spare time, he enjoys mountain biking and climbing, hiking, live music and his dogs.
Michael A Edwards, D.O.
Dr. Edwards is board certified by the American Board of Osteopathic Family Physicians. His undergraduate work was completed at Villanova University, where he was a member of the Villanova University football team. He received his Doctor of Osteopathic Medicine Degree from Nova Southeastern University. Dr. Edwards completed his residency with the St. Vincent’s Family Medicine Residency Program in Jacksonville, Florida. Prior to joining My Choice Housecalls, Dr. Edwards was the lead physician for Coastal Family Medicine in Orange Park. Dr. Edwards is a member of the American Osteopathic Association, Florida Osteopathic Medical Association and American College of Osteopathic Family Physicians. Dr. Edwards has been married for 16 years to his wife, Amy.
Melissa Runyan, VP Practice Operations
BA Psychology with minor in Spanish and Masters of Social Work, both at University of Louisville in Kentucky. Obtained her LCSW in 2003. Worked as a Clinical Social Worker providing school and home based counseling for children and families identified through the public school system with emotional and behavioral challenges. Also has experience working with victims of domestic violence. Committed to advocating for patients and serving the community. Enjoys traveling, participating in 5Ks with her husband and 2 boys, and spending time with family and friends.
Donna J. Cothren, MSN, ARNP, FNP-C
Prior to My Choice Housecalls, Donna served in a variety of nursing roles with St. Vincent’s Medical Center for nearly 15 years. She received her BSN from Jacksonville University and her MSN as a family nurse practitioner from South University in Savannah, Georgia in 2015. As a decorated family nurse practitioner she joined the My Choice Housecalls team in November 2015 and continues to thrive. Donna is very involved with her church and greatly enjoy studying and teaching the Bible.
Dawn Dust, MSN, ARNP, FN-C
Dawn has been with the My Choice Housecalls family from the beginning. She attended South University receiving her nurse practitioner degree. She has been a nurse practitioner for two years practicing specifically in house calls, as well as freelancing in a local Emergency Room. Dawn enjoys traveling around the world, vacationing with her family, learning new things, and meeting people along the way. She always looks forward to partnering with her patients and their families to provide exceptional healthcare in their home setting.
Regina I. Washington, ARNP, GNP-BC
Regina is ANCC Board Certified Gerontological Nurse Practitioner, and has been providing Long Term Care since 1987. She received her BSN from the University of Rochester School of Nursing, and her MSN at the Nazareth College of Rochester. She loves spending time with family, and is the proud mother of four and grandma of two. She also loves daily prayer, reading, cooking and listening to Jazz.
Julie Maly, ARNP, FNP-BC
Julie has been with My Choice Housecalls since 2016. Prior to MCHC she worked with hospice for 8 years, and has been doing home health since 2012. She received her BSN from the University of Florida in 1998, and her MSN from the University of Tampa in 2002. In her free time, she is dedicated to spending as much time as possible with her husband and three children.
Imee McGrath, ARNP, FNP
Imee has been with My Choice Housecalls since 2017. Prior to MCHC she worked with Coastal Dermatology and MD2U. She graduated from Jacksonville University in 1996, and went on to get her MSN as a Family Nurse Practitioner at Georgetown, graduating in 2014. She and her husband are the proud parents to 3 kids, Kristen, Megan and Michael.
Cavan Holley, NP-C
Cavan joined My Choice Housecalls in 2018. Prior to his work with My Choice, he worked as a registered nurse at Amedisys Home Health & Hospice and then as a nurse practitioner at Avalon Medical Center in Alabama. He also was a dedicated volunteer on multiple medical mission trips to Haiti. When he is not caring for patients, Cavan enjoys spending time with his wife Macy and dog Remi and family/friends, watching Alabama football and Arsenal Football Club, spending time outdoors, going to concerts, and traveling.
Dana Reynolds, ARNP
Dana joined My Choice Housecalls in 2018. Prior to her work at My Choice, she was with First City Hospitalist Group/Flagler Hospital in St. Augustine, FL, and a Locum Contractor at Christiana Care in both Wilmington, DE and Newark, DE. Dana loves reading, it is her absolute favorite thing to do. She is a Navy wife of 28 years, to the love of her life Gregory, and they have four beautiful daughters together. They love to travel the world. The military has given them a great opportunity to experience a variety of cultures, meet great friends and extended friends that have become family.
Jessica Volpe, ARNP-FNP
Jess joined My Choice Housecalls in 2018. Prior to My Choice, she had 15 years of experience at Ocala Regional Medical Center/West Marion Hospital and Orange Park Medical Center with diversified work in Med-Surg, ICU, Supervision, and Emergency Room. She received her MSN-ARNP from South University, her BSN from University of Central Florida, and both her ADN and AA from the College of Central Florida. She loves spending time with her two kids doing anything and everything from reading books, riding bicycle, swimming, and watching movies.
Brooke Brown, ARNP
Brooke is a Board Certified Advanced Registered Nurse Practitioner. Prior to joining My Choice Housecalls in 2018, she worked with Coastal Gastroenterology. She has a Bachelor’s of Science Degree in Health Science from the University of Florida. Brown went on to receive both her BSN and MSN from the University of North Florida. She is a member of Sigma Theta Tau International Nursing Honor Society, as well as the recipient of the Walgreens Fellowship Scholarship. Brooke enjoys spending time with her husband, daughter, and is a season ticket holder for the Jacksonville Jaguars.
Christy Carpenter, Practice Development Manager
Christy attended Jacksonville State University in Jacksonville, Alabama prior to relocating to Jacksonville, Florida. She has a wide range of experience in healthcare, including the areas of Phlebotomy, Pharmacy, Sleep Disorders and DME. She has also worked in Health Services Consulting, gaining experience in auditing, benefits and billing. She prides herself on providing excellent customer service with a focus on treating your family like her own. In her spare time, Christy is dedicated in her faith. She loves cheering on her boys baseball teams with her husband, and spending time with friends and family at the beach or camping.
Ally Perez, Practice Manager
Ally holds a Bachelor’s of Health Administration with a concentration in Gerontology. She has extensive volunteer experience working with seniors in the community. Her previous role was as a Social Services Director at a local skilled nursing facility. She is an animal enthusiast, enjoys baking, and loves spending time with her husband and daughter.
Tammie Johnson, Prescriptions Department, Team Lead
Tammie has been with My Choice Housecalls since 2016. She has nearly 20 years of experience in the medical field. She graduated as a Certified Medical Assistant from Concorde Career Institute, and then became a certified Pharmacy Tech from Everest. In her spare time, Tammie loves to read and is an aspiring author.
